Registered Users. If you are a licensed medical professional and/or medical facility located in the United States, and we conduct business in the State where you are located, you are eligible to become our customer and purchase products from us at wholesale. If you are eligible to become our customer, it is purely voluntary whether you choose to open a customer account with us or register for Website access to our customer account portal. Registered users of our portal, however, gain access to services and information that are not available on our public Website, and the ability to log in and out of their account history and review, download and print their transaction documentation maintained on the portal (including your transaction information, transaction history and transaction statement, or “T3 Statements”). To register for access to our portal, you must first open a customer account and voluntarily provide us with certain Personal Information and/or business information, including a designated email address that we can use to communicate with you.
